Red light from the sky: surface guidance for Radiotherapy 28 Feb 2020

The use of Surface Guided Radiation Therapy (SGRT) is rapidly becoming established as an accurate and reliable method for patient localisation and monitoring for multiple forms of radiation therapy treatment. This paper will present a single institution experience in the use of AlignRT for SGRT of linac-based stereotactic cranial radiosurgery (SRS) and stereotactic ablative body radiotherapy (SABR). At Beacon Hospital, AlignRT has been used since 2014 in the treatment delivery of cranial SRS and our series of over 300 patients is the largest in Europe.

An outline of the clinical workflow will be described as well as an analysis of the performance of SGRT. The use of AlignRT to localise and monitor the patient position during treatment delivery will be compared to standard linac-based imaging technologies (CBCT and planar imaging). In addition, the use of AlignRT for respiratory management during Radiotherapy will be described. Case studies will be presented for both SRS and SABR treatments that highlight the role of SGRT in allowing for enhanced quality in treatment delivery.
